Document Format (PDF) Files
This document is in "PDF" format. This is now a very common format which allows files to be created which contain all of the formatting of the original document. In other words, it looks the same on the screen and in print as the original form (including fonts, page numbers, etc.).
To read these, you will need a program called Adobe Acrobat Reader. If you do not have this program installed on your computer, you will need to download a copy. This can be done safely and free of charge from Adobe's Web-site:
